If you would like to be seen as a new patient in our clinic, please have a healthcare provider (Primary Care Physician/PA/NP, Physician Specialists, Chiropractor or Physical Therapist) fax a referral request with pertinent chart notes and imagining (MRI, xRay, CT Scan) to our office at (503) 385-8421. Our Physicians will review your case to ensure a treatment plan can be developed for your condition. You will then be contacted by our support staff and scheduled for your initial consultation.
