Salem Pain and Spine Specialists requires a referral from a treating health care provider prior to scheduling any new patient consultation. This requirement applies even if your insurance plan does not require a referral, as it helps confirm that SPSS can appropriately evaluate and treat the condition for which you are seeking care.
The physician-to-physician referral must include an evaluation of the specific area of pain you would like to be seen for, along with any relevant imaging and supporting medical records. This may include chart notes such as physical therapy reports, neurosurgery consultations, or records from previous pain management providers.
To help streamline the evaluation and treatment planning process, we ask that referrals focus on one primary area of pain at a time.
